Community support of the local health care system is vital for sustainability and growth. Navigating the transition from volume to value-based payment and population health within hospitals as well as with the community begins with setting a course based on a strong assessment and good planning. Learn how to create a healthier and more engaged community by working with The National Rural Health Resource center (The Center). The Center’s services may be selected as a single Community Health Needs Assessment service or a package of services, depending upon organizational or community need. In all cases, The Center’s assessment tools provide tangible outcomes designed to match an organization’s vision, mission and strategic goals.

Healthy Hospital, Healthy Community Bundle

The Center recommends the following services as you transition toward population health.

  • Six-page Community Health Needs Assessment 
  • Focus Group Sessions
  • Secondary Data Collection
  • Implementation Planning

Community Health Needs Assessment

The Center has assisted hundreds of hospitals and health councils during the past 25 years with community health needs assessments (CHNA) and marketing surveys. The assessments include:

  • Survey development
  • Random sampling and mailing list creation
  • Survey printing and distribution, tabulation and summary of results, including comparison with The Center’s rural database
  • Presentation of survey findings and recommendations to local governing board

$9,300 for distribution of 800 six-page surveys
$3,500 for distribution of 800 two-page surveys

Focus Group Sessions and Key Informant Interviews

This service can supplement your CHNA findings and/or secondary data collection to better understand the unique strengths and needs of your community. These sessions are an excellent way to capture those meaningful stories that can drive performance and quality. The Center develops questions, facilitates the conversations and documents findings to be presented with recommendations.                                    

$4,245 per four focus groups plus travel expenses
$2,750 per 12 interviews

Secondary Data Collection

Secondary data collection organizes local, state and national statistics that may affect health status, access and future health needs. The collection compares the community's county health status to same-state counties of a similar size, state and national averages, by gathering and synthesizing the following data into a report:

  • Demographics
  • Social & Economic Factors, Physical Environment
  • Clinical Care
  • Health Behaviors
  • Health Outcomes

$1,300 per analysis

Implementation Planning

The Center will guide your organization in working towards population health and value through translation of your CHNA findings into actionable work; offering support, facilitation and technical assistance in the development and achievement of your goals associated with population health and value. The Center facilitates an on-site session with the leadership team or community health council to identify community health priorities and offers technical assistance and guidance through regularly scheduled check-in calls to assure your success in meeting the needs of your community.

Strategic Planning

The overall goal of providing facilitated strategic planning is to support the rural health organization in reaching and meeting its mission. A strategic planning facilitation will include an on-site planning session with the board of directors and hospital or network leaders to develop strategic objectives. To build capacity with the leaders and board, an introduction of the balanced scorecard methodology will be provided. The outcome of the planning event is a balanced strategy map.

  • Secondary data analysis

Basic Community Snapshot Bundle

The Center recommends the Healthy Hospital, Healthy Community Bundle listed above as you transition toward population health, however, this bundle will give you a snapshot of your community health needs, as well as meet the IRS requirement for non-profit hospitals:

  • Key Informant Phone Interviews
  • Implementation Planning via Webinar
