Nomination Period Open for the MBQIP Spirit Awards

The Federal Office of Rural Health Policy (FORHP) is accepting nominations for public recognition of State Flex Coordinators (SFCs) who exemplify the collaborative and innovative spirit of the Medicare Beneficiary Quality Improvement Project (MBQIP). The purpose of the MBQIP Spirit Award is to recognize state Flex Coordinators (SFCs) who have made remarkable strides in innovations, improvements, and the advancement of MBQIP objectives in their state.

Please note that SFCs can nominate themselves, individuals on their staff, or other SFC colleagues. In addition, members of the Flex team, Technical Assistance Service Center (TASC) staff, Rural Quality Improvement Technical Assistance (RQITA) staff, and the Flex Monitoring Team (FMT) can also nominate individuals.

How to Submit an Award Nomination

The 2020 nomination period has closed. Nomination packages must be addressed to the MBQIP Coordinator (Natalia Vargas) and include the following four (4) elements:

  • Element 1-Nominator Information: Name, state, position, role
  • Element 2-Nominee Information: Name, state, position, role
  • Element 3-Justification Statement*: Explanation of why you are nominating this person and how he/she exemplifies the collaborative and innovative spirit of MBQIP.
  • Optional Element 4-Optional Photograph(s): If you would like to share a picture of the nominee with your nomination package, please submit in a jpeg file attachment, and include a “release statement for photographs” within Element 1, authorizing FORHP to use the photograph(s) for the purposes of the Flex Program, MBQIP, and MBQIP Award Ceremony.

Specific Instructions

The justification statement* is limited to a maximum of 250 words. Nominations must clearly articulate why the person is being nominated, and specific examples of how the person embodies the MBQIP spirit through collaboration and innovation in their work. No attachments or hyperlinks within the document will be viewed/opened. Any nominations that do not adhere to the requirements outlined above and are not submitted by the deadline will not be considered.

How are Awardees Selected? FORHP uses specific scoring criteria to conduct an objective review of each nomination package. More specifically, each nomination will be scored on a 10-point scale.

When Will FORHP Announce Awardees? Awards will be presented at the 2020 Virtual Flex Reverse Site Visit.
