The road to population health depends heavily on our ability to overcome the most vexing barriers to access. When community members struggle with transportation, insufficient or no insurance, health literacy and other challenges, they can’t access the services that networks work so hard to provide. While there are no easy fixes, even incremental improvements can over time make significant improvements in a community and the lives of the people who live there.

During this summit, you’ll work with other network leaders from across the country to brainstorm and share best practices for addressing barriers to access. You’ll also learn strategies for creative thinking from an expert in the field of innovation.

Through structured, facilitated discussions, you’ll have the opportunity to focus on the most pressing barrier to access for you and your community, and will walk away with new ideas and renewed energy around your work.

Objectives:

  • Explore the role of networks in addressing health care access
  • Build creative thinking skills
  • Generate ideas and share best practices for improving health care access
  • Contribute to a Summit report that will be used as a reference document for network leaders across the country

The Network TA Team encourages participation from Network Directors, Project Directors/Mangers, Care Coordinators and any other networking staff who is working to improve access to care.
