Care Coordination is essential for networks as they transition from pay per procedure to reimbursing for value. As a team-based and integrated approach to population health management, Care Coordination across the continuum addresses many Social Determinants of Health factors that play a role in health outcomes. Care coordination effectively integrates the patient experience across a continuum of services which include primary care, hospital, behavioral health, social services, rehab, long-term care and home care. In the new value models, Care Coordination is key to both successful patient care outcomes and financial success.

Through this interactive workshop, participants will learn about the components of Care Coordination and apply them to meet their networks goals. The aim of this workshop is to learn, create and/or improve upon a Care Coordination model that specifically addresses your network and community's needs while including infrastructure for your team development and workflow processes. By the end of day two, participants will have gained confidence in their own network projects, new insights and built new relationships with network peers.
