The 2016 Network Care Coordination Workshop was open to grantees of the Network Development, Network Planning, Health IT Workforce, and Allied Health grant programs of the Federal Office of Rural Health Policy (FORHP). This workshop reviewed how social determinants of health affect the coordination of care in our communities. In view of this approach, the workshop addressed the role community health workers play in combatting these areas as well as the use of technology in coordinating care.
