This is the third of five sessions in the Value-Based Care (VBC) Webinar Series. The session will discuss best practices for transitioning to VBC models and being successful with payment system transformation and population health.

  • Hear how payors and health plans define quality in an ambulatory setting
  • Hear how commercial payors incentivize hospitals based on care measures
  • Learn successful strategies and risk adjustment opportunities 

The audience includes DRCHSD Program participating rural hospitals, clinics, and federally qualified health centers (FQHCs) within the eight-state Delta Region as well as state offices of rural health, hospital associations, the Federal Office of Rural Health Policy, among others.
