Jackson Medical Center (JMC) executive teams shares their experience in completing a quality improvement project targeting care management and how they plan to prepare for value-based purchasing. JMC team discuss how they are implementing care management best practices and learning how to further support the care manager’s role. The care manager’s role is critical to not only direct patient care, but is key to population health management of the future. As the health care moves towards population health, the care manager will become a key community liaison between the patient/family, PCPs/Hospitalists and staff.
