Speakers will share successes and lessons learned from Guadalupe County Hospital’s (GCH) SRHT quality improvement (QI) project. Speakers will discuss next steps to improve transition of care processes and prepare for better community care coordination, as well as strategies in continuing to move towards population health management and the new health care delivery models of the future.
