This panel shares the challenges that rural hospitals are facing with regards to population health planning. Union General Hospital shares how they used results from their community health needs assessment (CHNA) to target and initiate a local wellness program. Illinois Critical Access Hospital Network (ICAHN) Director discusses steps that Illinois hospitals took to respond to Accountable Care Act (ACA) and prepare for value-based reimbursement.
