Care management programs are critical to improving the health and wellness of primary care patients. By implementing one of the multiple models of care management services, primary care providers — including those in rural health clinics (RHCs) — can also capture additional revenue for managing eligible beneficiaries within primary care practices.
This two-part webinar will share care management program best practices including program specifics for patients and providers, billing and coding, documentation and care plans, and continuity of care and care transitions.
