Care management programs are critical to improving the health and wellness of primary care patients. By implementing one of the multiple models of care management services, primary care providers — including those in rural health clinics (RHCs) — can also capture additional revenue for managing eligible beneficiaries within primary care practices. 

This two-part webinar will share care management program best practices including program specifics for patients and providers, billing and coding, documentation and care plans, and continuity of care and care transitions. Hospitals, clinics and providers will improve their understanding of each program, the specific requirements of each program and the reimbursement opportunities available to them. 

 

Part I: Chronic Care Management Program 101

A chronic care management (CCM) program is a critical component in improving the health and wellness of primary care patients. Additionally, a CCM is a financial opportunity for provider organizations, which can realize additional revenue for managing eligible beneficiaries within primary care practices, including RHCs. This presentation will share CCM program best practices including program specifics for patients and providers, billing and coding, documentation and care plans, and continuity of care and care transitions. Hospitals and clinics will gain a better understanding of the requirements of the CCM program.
