2018 Rural Health Care Improvement Collaboration

The purpose of this collaborative meeting is to create a shared learning environment that supports innovations and discovery of successful rural health improvement and program implementation across FORHP programs. The meeting will engage participants from two rural health care improvement grant programs: the Medicare Rural Hospital Flexibility (Flex) Program in the FORHP Hospital State Division (HSD) and the Small Health Care Provider Quality Improvement Program (SHCP) in the FORHP Community-Based Division (CBD). In addition to program-specific meeting objectives, this meeting will also provide participants with an enhanced opportunity to make connections, share best practices and gather lessons learned from fellow rural programs across the country that are focusing on improving rural health care services.

SHCP Program Partnership Meeting

FORHP CBD's Small Health Care Provider Quality Improvement Program (SHCP) Partnership Meeting, will convene organizational representatives from the program’s current awarded grant projects to create a shared learning environment that supports innovations and discovery of successful rural program implementation. The meeting will focus on information sharing, best practices and reflections on Year Two project implementation and anticipated Year Three activities and results. Peer project sharing and learning opportunities will include the opportunity for each quality improvement project to present their unique experiences to grant program peers and explore rural quality improvement innovations and strategies among fellow grant recipients, and other rural partners and stakeholders. We look forward to your attendance at this educational and informative event. 

SHCP Program Partnership Meeting Learning Objectives

  • Discover ways to communicate the value of your rural quality improvement project
  • Identify opportunities to implement best practices and lessons learned through shared experiences from fellow awarded projects
  • Strengthen the leadership, implementation and targeted outcomes of your rural quality improvement project
  • identify opportunities for collaboration through networking and making connections with others from programs across FORHP

Audience

The intended audience for the SHCP Program Partnership Meeting includes the designated Project Director and up to one additional organizational representative from the program’s current awarded grant recipients. FORHP project officers and technical assistant consultants from the Georgia Health Policy Center (GHPC) and Stratis Health’s Rural Quality Improvement Technical Assistance (RQITA) will also attend. If SHCP grantees would like to have more than two attendees, please contact your project officer at FORHP for approval. Additionally, all guidelines for SHCP project presentations will be forthcoming and provided soon from your Program Coordinator.
