Apply for a Financial or Quality Small Rural Hospital Transition (SRHT) Project!

The SRHT project will select and support nine (9) hospitals for on-site technical assistance that helps them to transition to a value-based system and prepare for population health. Applicants may select either a financial operational assessment (FOA) or quality improvement (QI) project. At least three (3) hospitals will be selected for FOAs and at least three (3) hospitals for QI projects.

The 2018-2019 SRHT application opens September 26 and closes midnight on October 24, 2018. The SRHT application page provides a preview to allow applicants to prepare in advance of the online release date and includes helpful hints for submitting a successful application. Applicants must be willing and able to meet hospital readiness and participation requirements.

Eligible hospitals must be small rural hospitals located in a rural community, as defined by FORHP, persistent poverty county or a rural census tract of a metro persistent poverty county, and have 49 staffed beds or less, as reported on the hospital's most recently filed Medicare Cost Report. Hospitals may be for-profit or not-for-profit.
