Network Self-Assessment Tool

The Network Self-Assessment Tool is designed to assist rural health networks in examining and building organizational leadership capacity. Created for grantees of the Rural Health Network Development Planning Program, the tool was developed collaboratively by Rural Health Innovations (RHI), the National Cooperative of Health Networks Association (NCHN) and the Federal Office of Rural Health Policy (FORHP). The Network Self-Assessment Tool provides networks and their stakeholders with information on areas of strengths and challenges within their projects.

This tool was updated in November 2015.
