How Technology is Impacting Population Health

Improving population health can be complex and is an area in which technology can make a difference. Communicating with multiple providers, caring for patients in their home, sharing information amongst providers, and understanding the impact of our work are some of the areas that technology can play a significant role. The purpose of this cohort call is to introduce ways you can utilize technology, and hear from grantees how technology is being used in their project work. 

Joe Wivoda, Senior Director of Healthcare with Analysts and ACS Group, will moderate our conversation as our Network TA technology subject matter expert (SME). He will begin the call with a brief presentation on ways technology is helping improve population health and care coordination via data analytics and dashboards, telehealth, remote patient monitoring, home health monitoring and data sharing. Grantees should come prepared to share their technology plans and experience. 

Objectives:

  • Learn ways in which technology is being used to improve population health
  • Gain insights about grantees' use of technology
  • Gain insights of today's technology trends that are driving population health management
  • Identify areas of interest amongst grantees that may warrant further discussion
