Sustainability of rural health networks and projects is determined by how productive the network is in achieving its objectives and goals, and by how successfully it has positioned itself for the future in terms of both operational stability and financial viability. The Sustainability Plan is the final deliverable for the Rural Health Information Technology (HIT) Workforce Program grant. This webinar introduces the Sustainability Plan Toolkit, which RHI developed for HIT Workforce grantees based on our knowledge and experience in working with management tools and rural health networks. The Sustainability Plan Toolkit includes a guide, a template and a sample action plan. The presentation provides background information on sustainability planning and reviews the components of the toolkit to support grantees in effectively meeting the Sustainability Plan deliverable.
