SRHT Project Eligibility

Small rural hospitals located in a rural community, as defined by FORHP, persistent poverty county or a rural census tract of a metro persistent poverty county, and have 49 staffed beds or less as reported on the hospital's most recently filed Medicare Cost Report.  Hospitals may be for-profit or not-for-profit.

To Confirm Eligibility

  1. Hospitals must have 49 staffed beds or less as reported on the most recent Medicare Cost Report
  2. Hospitals must be located in a federally designated persistent poverty county (PPC).
    a.  To confirm if your hospital is located in a PCC, download the USDA ERS County Typology Codes to check your county and state.
  3. Hospitals must also be located in a rural county or a rural census track of a PPC.
    a.  To confirm if the county is designated as rural, check the USDA ERS County Typology Codes. This list defines counties as metro and non-metro. Non-metro counties are considered rural.  
    b.  If the county is defined a metro, then confirm if the hospital is located in a rural census track by checking the location in HRSA’s Rural Health Grants Eligibility Analyzer.
