SRHT Project Eligibility

Small rural hospitals located in a rural community, as defined by FORHP, persistent poverty county or a rural census tract of a metro persistent poverty county, and have 49 staffed beds or less as reported on the hospital's most recently filed Medicare Cost Report. Hospitals may be for-profit or not-for-profit, including faith-based. Hospitals in U.S. territories as well as tribally operated hospitals under Titles I and V of P.L. 93-638 are eligible to apply.

To Confirm Eligibility

  1. Hospitals must have 49 staffed beds or less as reported on the most recent Medicare Cost Report
  2. Hospitals must be located in a federally designated persistent poverty county (PPC).
    To confirm if your hospital is located in a PCC, download the USDA ERS County Typology Codes to check your county and state.
  3. Hospitals must also be located in a rural county or a rural census track of a PPC.
    To confirm if the county is designated as rural, check the USDA ERS County Typology Codes. This list defines counties as metro and non-metro. Non-metro counties are considered rural.  
    If the county is defined a metro, then confirm if the hospital is located in a rural census track by checking the location in HRSA’s Rural Health Grants Eligibility Analyzer.
