This SHIP Technical Assistance (TA) webinar will explore software for population management. Small, rural hospitals can use software and training for analysis of population health needs by chronic disease or geographic location for care management programs.

By attending this SHIP TA webinar, participants will be able to:

  • Identify how hospitals can use information technology (IT) tools to manage population health
  • Understand the important steps to implement population health tools
  • Communicate the importance of population health and the importance of population health management tools as a SHIP allowable investment
