Best Practices to Work with Community Partners on Population Health Initiatives

This session will focus on learning best practices to engage community partners in the transition to value-based care and hearing about successful models for engaging community partners in population health initiatives.

The audience includes DRCHSD Program participating rural hospitals, clinics, and federally qualified health centers (FQHCs) within the eight-state Delta Region as well as state offices of rural health, hospital associations, the Federal Office of Rural Health Policy, among others.
