In this webinar, Terry Hill, Executive Director and Senior Advisor for Rural Health Leadership and Policy at the National Rural Health Resource Center addresses the importance of care coordination as your hospital or clinic moves forward into Value-Based Payments. He is joined by Debra Laine, Sr. Program Specialist at The Center, who will explain foundational elements of an effective care coordination program so you as an administrator will know how to support the effort.
