Moving the needle on health outcomes requires moving beyond the walls of the health care system out into the community where decisions that affect health are made on a daily basis. This webinar will share the lessons learned from the 12 years of the Heart of New Ulm Project as it transitioned from a research project to a community-owned and driven community health improvement initiative. Partnering with non-traditional organizations that have a role to play in the health and wellness of the community is critical to moving farther upstream in primary prevention. Pairing programs with policy, systems, and environmental change provides the foundation for long-term sustainable change that supports healthy lifestyle behaviors and improved health outcomes.
