McKenzie Health System, Sandusky, Michigan

McKenzie Health System (MHS), located in Sandusky, Michigan, received recognition for working on key projects that will ensure their viability in the future, while improving the health care outcomes of their population. One of the projects was to move toward the Patient Centered Medical Home (PCMH) model to develop a deeper relationship with patients and develop skills to improve patient health through evidence-based medicine techniques to manage chronic disorders.

 

McKenzie Health System executives receive CAH Recognition certificate from the Michigan Flex Program

Positive Outcomes:

  • Launched a patient portal, “My Record of Health”.
  • Supported wellness lunch-and-learns, fitness programs, and lifestyle workshops to promote healthy lifestyle choices.
  • Implemented a new electronic health record (EHR) that supported tracking and reporting.
  • Participated in the National Rural Accountable Care Organization (NRACO).

Top Accomplishments:

Upon receipt of the recognition, Steve Barnett, CEO, said, “The award is both recognition and affirmation of the changes we are going through. All of us at McKenzie Health System have been working hard to shift how we deliver care toward value and away from volume and this award helps motivate our staff to continue down this path.”
