Carondelet Holy Cross Hospital, Nogales, Arizona

Carondelet Holy Cross Hospital (CHCH), located in Nogales, Arizona, was recognized for innovation in emergency medical services (EMS). In 2014, health care providers serving residents of Santa Cruz County, Arizona, came together to create the Community Healthcare Integrated Paramedicine Project (CHIPP).

Positive Outcomes:

  • CHCH partnered with Rio Rico Medical & Fire District to maximize successes from federal Rural Health Services Outreach Program grant for CHIPP in 2015
  • Home visits from Community Paramedics showed a reduction in emergency department  visits and hospital readmissions
  • Active collaboration with state Flex Program

CHCH is a proud partner in CHIPP and continues to find innovative models of care to provide quality health care for all Santa Cruz County communities.
