Estes Park Medical Center (EPMC), located in Estes Park, Colorado, was recognized for its work in the areas of care coordination, care management, patient access and prevention and wellness projects. EPMC joined with the historic Stanley Hotel, the town of Estes Park and University of Colorado Health in a public-private partnership to form a wellness center that provides an array of services to the community to address physical fitness, metabolic fitness, nutritional fitness, sleep, stress and quality of life/life satisfaction.

  • Partnered with local schools by enrolling students in wellness model and following up over time
  • Addressed Community Health Needs Assessment (CHNA) findings

Top Accomplishments:

The vision of the project is for Estes Park to be a destination for wellness and a community that embraces the culture and principles of wellness. Brian Herwig, Estes Park Medical Center CEO, said, “We are honored to have received this recognition and it illustrates our commitment to innovative value-based health care initiatives and our goal of improving the health and wellness in our community.”
