Health care delivery drives only 20 percent of health outcomes. The next step in your Path to Value project may be to reach out and engage your community partners. To truly have an impact on your population’s health you must address the behavioral and social determinants of health - including income, living environment, employment status, and access to transportation and healthy food. This webinar will give you best practices and strategies to identify partners as well as tips to engage them and sustain those partnerships.

Objectives:

  • Learn a process to identify community resource partners.
  • Determine how to engage partners.
  • Select ways to sustain partnerships.
