Care management programs are critical to improving the health and wellness of primary care patients. By implementing one of the multiple models of care management services, primary care providers — including those in rural health clinics (RHCs) — can also capture additional revenue for managing eligible beneficiaries within primary care practices.
This two-part webinar will share care management program best practices including program specifics for patients and providers, billing and coding, documentation and care plans, and continuity of care and care transitions. Hospitals, clinics and providers will improve their understanding of each program, the specific requirements of each program and the reimbursement opportunities available to them.
The Advanced Primary Care Management (APCM) Program is a health care initiative designed to improve the delivery, coordination and outcomes of primary care services. The APCM typically focuses on enhancing the quality of care while managing costs and improving patient and provider experiences. Programs like APCM are often implemented in health care systems aiming to transition from fee-for-service models to value-based care. While Medicare does not explicitly label its initiatives as "APCM," the following features align with the APCM concept: team-based care, integration of behavioral health, use of health information technology, focus on preventive care and chronic disease management, and value-based payment mechanisms.
