Ultimately, the population health movement is about caring for the health needs of the community. Financial distress, however, causes many rural health care organizations to focus on generating cost savings instead of population health activities. This webinar will address the question - is there a way to utilize population health concepts and community engagement to drive down the global cost of care, while also enhancing the local organization’s portion of the cost incurred?

Upon conclusion of this webinar, participants will be able to
  • Apply population health practices to drive local volumes for existing patients.
  • Recognize how a community focus can lead to increased market share and volumes.
  • Differentiate how total cost of population health care can decrease while the local providers' volumes and revenues increase.

FORHP-funded programming increases health care access, strengthens health networks, and improves quality of care for Critical Access Hospitals, small rural hospitals, Rural Health Clinics, and other rural healthcare organizations.
