As we near the mid-way point of the Rural Healthcare Provider Transition Project's (RHPTP's) fifth year, we would like to share the progress and lessons learned from the 2024-2025 cohort.  

 

Please join us to hear updates and participate in discussion on the following: 

  • We will share updates about the current cohort of hospitals and clinics, webinars scheduled for this year, and information about the current learning collaborative, Advancing Value-Based Care: Leading from the Middle.
  • Our project specialist will share measurable outcomes from the FY2023 cohort.
    Stroudwater Associates will join us to share trends in the cohorts regarding readiness to move to an alternative payment model.
  • We would like to hear from you about how the response or participation in advanced alternative payment models (or other models) is changing in your state across rural hospitals and clinics.
