Community Health Workers Toolkit

Community health worker (CHW) is an umbrella term describing health and social service workers who are members of the community and who assist individuals and communities to adopt healthy behaviors. CHWs, and their duties, are as diverse as the communities they serve. This toolkit can assist organizations and communities in evaluating opportunities for developing a CHW program, including resources and best practices. The eight modules feature focus areas such as program models, training approaches and and measuring impact.
