CentraCare Health - Paynesville

CentraCare Health – Paynesville is a 30-bed critical access hospital (CAH) located in Paynesville, Minnesota. CentraCare Health – Paynesville has an unchanging dedication to offer excellence in every area of their health care system.

In 2016, the hospital began participating in Rural Health Innovation’s Integrative Behavioral Health Program funded by the Minnesota Department of Health, Office of Rural Health and Primary Care.

CentraCare Health – Paynesville finished their fourth and final Recommendation Adoption Progress (RAP) interview in July 2018. Their outcomes and measures are currently being reviewed and summarized.

Project Goal Enhance provider awareness and improve processes for tracking referrals and care coordination 
Target Population 11-30 years old (and their families) at risk of behavioral crisis
