Ridgeview Le Sueur Medical Center

Ridgeview Le Sueur Medical Center is a 25-bed critical access hospital (CAH) located in Le Suer, Minnesota. They stand by their mission to provide community-based health care of highest quality in an environment that fosters dignity, respect and excellence

In 2017, the hospital began participating in Rural Health Innovation’s Integrative Behavioral Health Program funded by the Minnesota Department of Health, Office of Rural Health and Primary Care.

Ridgeview Le Sueur Medical Center will have their first quarterly Recommendation Adoption Progress (RAP) interview in October 2018. During the four RAP interviews, information on the progress of project implementation and project outcomes will be gathered. 

Project Goal

Reduce 30-day readmissions to the emergency department (ED)/urgent care 

Target Population Patients 18-60 years old with a chief complaint of depression, anxiety, self-harm, psychiatric and drug/alcohol misuse
