Sanford Jackson Medical Center

Sanford Jackson Medical Center is a 20-bed critical access hospital located in Jackson, Minnesota. They are dedicated to providing high quality, affordable and compassionate health care services for families throughout Jackson County, Minnesota and the surrounding area. 

In 2016, the hospital began participating in Rural Health Innovation’s Integrative Behavioral Health Program funded by the Minnesota Department of Health, Office of Rural Health and Primary Care.

Project Goal Prevent behavioral health (BH) crisis-related readmissions within 60 days of another visit 
Target Population Adults (18-60 years old) in a BH crisis

  • Decreased number of emergency department (ED) clinic visits 
  • Decreased number of patients presenting to the ED with BH disorder, depression, anxiety, or substance abuse as primary encounter diagnosis 
  • Decreased percentage of BH patients discharged from ED to an inpatient BH facility
  • Decreased number of 72 hour holds placed for residents of Jackson County 
  • Decreased median length of stay (time in ED) for patients with BH primary encounter diagnosis
