Population health is foundational in the “Triple Aim” of better care, healthier people and communities, and smarter spending. As a population health leader in your organization, your understanding of the multiple elements and benefits involved in improving the health outcomes of your community or defined population is critical. This webinar will explore ways to help staff understand this concept, how their work contributes, and inspire them to work together to improve the health of the population you serve.
