Population Health: Readiness Assessment
The primary goal of this assessment is to provide rural health organizations users with a systems-based approach to assess their unique strengths and needs for transitioning toward population health. The assessment is organized into the seven Baldrige categories to ensure a holistic approach for success. Users are encouraged to complete the assessment multiple times to monitor progress and the access updated resources within the Population Health Toolkit to guide their journey toward population health.
