Patients, Partners, and Community Resources

Engagement of patients, partners, and the community is the underpinning of successful population health improvement strategies. It is where the rubber hits the road, where you get going. Meaningful engagement can be a challenge and takes time and resources. Organizations' roles and the community’s health needs may vary over time, but the effort in identifying how organizations coordinate and collaborate is critical to successful partnerships and population health improvement.
