The monthly e-newsletter delivers resources, educational events, and targeted technical assistance opportunities to help strengthen the foundation in key elements of value-based care for qualified small rural hospitals and certified rural health clinic.

Applications are being accepted for the 2023-2024 cohort of the Rural Healthcare Provider Transition Project (RHPTP).

Small rural hospitals and certified rural health clinics selected to participate in RHPTP receive one year of focused technical assistance (TA) designed to strengthen their organization's understanding of the key elements of value-based care. Project focus areas include quality, efficiency, patient experience, and safety. Five applicants are selected each year to receive virtual and on-site TA.

Meet the RHPTP Technical Assistance Consultants

The Rural Healthcare Provider Transition Project (RHPTP) supports small rural hospitals and certified rural health clinics in strengthening their organization's understanding of value-based care (VBC) and teaches strategies they can use to be effective participants in a health care system focused on value.

RHPTP's quality technical assistance (TA) is provided by Stratis Health, a nonprofit organization leading collaboration and innovation in health care quality and safety and serves as a trusted expert in facilitating improvement for people and communities. Stratis consultants provide TA in the areas of efficiency, building infrastructure to embed improvement into everyday practice, patient experience, or safety of care. An initial organizational quality assessment helps to identify a quality focus area for each project participant.  That assessment is followed by regular coaching calls to gauge organizational alignment, embed and improve programs and organizational culture, identify target opportunities for improvement, develop SMART goals and work plans, facilitate action planning, identify resources, and celebrate successes.

Financial and operational TA is provided by Stroudwater Associates, an organization of national leaders in health care advisory who focus on improving organizational strategic, operational, and financial capabilities. Stroudwater consultants identify strategies, provide coaching, and develop tactics to address operational cost-efficiency opportunities in preparation for participating in VBC arrangements. An introduction of Stroudwater’s Value-Based Care Transition Framework is provided, along with an in-depth assessment to determine readiness for assuming financial risk, a population health readiness assessment, and a health equity dashboard crafted for each organization’s specific service area. Consultants also facilitate an action planning session for each organization to assist in future improvement efforts. Through the RHPTP's focused TA, health care organizations across the country are successfully becoming more effective participants in a health care system focused on value. This webinar will offer participants a deeper dive into the purpose and goal of the Rural Healthcare Provider and Transition Project (RHPTP).  Attendees will glean detailed information regarding project eligibility, benefits of participation, educational opportunities, project timeline, and the application and selection process for the 2023-2024 cohort.  In addition, RHPTP's technical assistance consultants will provide an overview of the services they offer and past RHPTP participants will share their experiences in the project.

RHPTP HELP Webinar: Hospital at Home for Rural Communities

Speakers: Dr. Margaret Paulson, Mayo Clinic College of Medicine and Science, and Maren Nordenberg, Mayo Clinic Health System

Telehealth's explosion during the pandemic made it possible to receive health care from nearly anywhere.  Hospital at home has quickly evolved from doctors making house calls to a hybrid telehealth model, blending bedside care with the safety of 24/7 virtual access to a health care team from the comfort of your home.  This care delivery model not only benefits the patient by making high-quality health care more accessible, it has also been shown to reduce costs, improve outcomes, and enhance patient experience. While hospital at home has primarily been instituted in urban spaces, this webinar will explore how rural communities have much to gain from this care delivery model as well.
