The Federal Office of Rural Health Policy (FORHP) improves rural health care through a variety of grant programs that support local health providers and patients. FORHP-funded programming increases health care access, strengthens health networks, and improves quality of care for Critical Access Hospitals, small rural hospitals, Rural Health Clinics, and other rural healthcare organizations.
The Advanced Primary Care Management (APCM) Program is a health care initiative designed to improve the delivery, coordination and outcomes of primary care services.
