Quality improvement alignment between the hospitals and clinic(s) is vital to value-based payment. By coordinating efforts across care settings, organizations can achieve better overall patient care and improve performance on VBP related quality metrics. This webinar will explore key clinical quality measures utilized in a variety of value-based payment programs and discuss opportunities to ensure quality improvement activities are aligned across the organization. Participants will also learn about actionable strategies to support sustainability of improvement efforts across the organization.
