Benefits of Participation

Small rural hospitals and certified rural health clinics that participate in the Rural Healthcare Provider Transition Project (RHPTP) receive a range of virtual and on-site technical assistance (TA) designed to strengthen each organization’s understanding of value-based care (VBC) and share strategies they can use to be effective participants in a health care system focused on value.

TA offered to the project’s five annual participants includes:

• In-depth comprehensive consultations and coaching with subject matter experts to support the implementation of best practices and adoption of transition to value-based care strategies to prepare organizations for alternative payment models. Consultations may target clinical areas of efficiency, quality, patient experience, or safety of care.
• An assessment of organizations' readiness to assume financial risk.
• Participation in a virtual peer-to-peer learning collaborative intended to support project implementation and sustainability.
• A Health Education Learning Program (HELP) webinar series, with sessions focusing on advanced payment models and/or other related VBC topics.
• Participation in post-project Recommendation Adoption Progress (RAP) calls which document progress, successes, and provide additional follow-up and support for project implementation.
