The monthly Timely Transitions newsletter, funded by the Federal Office of Rural Health Policy (FORHP), Health Resources and Services Administration (HRSA), includes Hospital Spotlights that highlight examples of small rural hospitals that successfully implemented best practices to improve performance and adopted transition strategies to prepare for population health.
