2020-2021 RHPTP Selected Hospitals and Clinics

On behalf of the Federal Office of Rural Health Policy (FORHP), the National Rural Health Resource Center (The Center) is excited to announce the 2020-2021 small rural hospitals (SRH) and certified rural health clinics (RHC) chosen to participate in the Rural Healthcare Provider Transition Project (RHPTP):

  1. County of Lawrence Health Department, Lawrenceville, IL, (Certified RHC)
  2. Guadalupe County Hospital, Santa Rosa, NM (Small Rural PPS Hospital)
  3. Lincoln County Medical Center, Ruidoso, NM (Small Rural CAH)
  4. Sudan Medical Clinic, Sudan, TX (Certified RHC)

Organizations were selected based on leadership, commitment, and organizational preparedness for transitioning to alternative payment and care delivery models of the future.

This project is supported by the Health Resources and Service Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) under grant number U5ERH39345 as part of a financial assistance award totaling $800,000 (0% financed with nongovernmental sources).
