Boon County Hospital (BCH) demonstrated excellence in quality initiatives with documented outcomes. BCH actively participated in the Partnership for Patients as well as many other quality initiatives.

Speare Memorial Hospital (SMH) implemented Lean and Six Sigma training methods, which have fostered a hospital-wide performance improvement infrastructure. SMH also established a Transition Care Program to allow care transition nurses to continue to follow their patients' post-hospital care at their homes.

Quality improvement services assist selected health organizations participating in the Delta Region Community Health Systems Development (DRCHSD) Program in evaluating their care management and transitions of care processes to establish planning priorities that improve operational efficiency and quality performance.

Revenue Cycle and Physician Practice Management services evaluate the business and clinical operations of selected healthcare organizations participating in the Delta Region Community Health Systems Development (DRCHSD) Program. Services assess areas that directly affect the operational and financial management of an organization, and impact clinic operations, provider productivity, and profitability.

Employee engagement services assist leaders that participate in the Delta Region Community Health Systems Development (DRCHSD) Program with retaining employees that are passionate about their work and committed to their organization. Leaders receive coaching and guidance to improve employee retention and satisfaction.

The Emergency Medical Services Assessment identifies opportunities for improvements — including gaps in clinical services and the need for improved processes to enhance the coordination of services — at health care organizations participating in the Delta Region Community Health Systems Development Program.

Community care coordination (CCC) activities consist of events and workshops, check-in calls, learning collaboratives, training bootcamps, and an annual summit.

Eligible applicants include small rural hospitals (CAHs and PPS acute care facilities up to approximately 100 beds) RHCs, and other healthcare organizations located in Delta Regional Authority (DRA) designated counties and parishes.
