Program Activities

The Delta Region Community Health Systems Development (DRCHSD) Program offers a variety of technical assistance and consultation services through a three-year participation cycle to assist rural health leaders and providers with implementation of best practices and evidenced-based models that improve performance and enhance care coordination to develop a local system of care. DRCHSD Program services fall under two areas:

  • Community care coordination
  • Healthcare organization consultations

Community care coordination in conjunction with consultation services that promote performance improvement initiatives help leaders to:

  • Position the organization for future success
  • Participate in, and benefit from, value-based care
  • Prepare for population health
  • Sustain access to local healthcare services
 

Community Care Coordination

The Delta Region Community Health Systems Development (DRCHSD) Program provides community care coordination (CCC) planning and strategic technical assistance to all program participants — with the goals of aligning local services with community needs, improving health outcomes and addressing social determinants of health.
Community care coordination (CCC) activities consist of events and workshops, check-in calls, learning collaboratives, training bootcamps, and an annual summit.

Consultations

The Delta Region Community Health Systems Development (DRCHSD) Program provides comprehensive technical assistance to participating health care organizations.
Financial and operational consultation services assess organization-wide operations at the department level of selected health care organizations participating in the Delta Region Community Health Systems Development (DRCHSD) Program to establish strategic priorities.
Quality improvement services assist selected health organizations participating in the Delta Region Community Health Systems Development (DRCHSD) Program in evaluating their care management and transitions of care processes to establish planning priorities that improve operational efficiency and quality performance.
Revenue Cycle and Physician Practice Management services evaluate the business and clinical operations of selected healthcare organizations participating in the Delta Region Community Health Systems Development (DRCHSD) Program. Services assess areas that directly affect the operational and financial management of an organization, and impact clinic operations, provider productivity, and profitability.
Employee engagement services assist leaders that participate in the Delta Region Community Health Systems Development (DRCHSD) Program with retaining employees that are passionate about their work and committed to their organization. Leaders receive coaching and guidance to improve employee retention and satisfaction.
The Delta Region Community Health Systems Development (DRCHSD) Program offers program participants a variety of services in the areas of workforce and leadership.
The Telehealth Assessment identifies opportunities for telehealth implementation or expansion at health care organizations participating in the Delta Region Community Health Systems Development (DRCHSD) Program. 
The Emergency Medical Services Assessment identifies opportunities for improvements — including gaps in clinical services and the need for improved processes to enhance the coordination of services — at health care organizations participating in the Delta Region Community Health Systems Development Program.
