DRCHSD Program Activities and Timeline
The DRCHSD Program uses a multi-faceted approach over three years to assist leaders and providers with the implementation of Program activities. Services are delivered in a systematic order for effectiveness. Each year contains projects that build internal capacity first and then expand over time. Program activities fall under two areas: Community Care Coordination (CCC), and health care organization consultations.
Community Care Coordination (CCC)
CCC activities include:
- Community Champion (CC) learning collaboratives
- Bootcamps
- Community Champion Check-in Calls
- CCC Onsite Events
- CCC Strategy and Sustainability Development
Health Care Organization Consultations
Health Care Organization consultation activities include:
- Financial and Operational Assessment (FOA)
- Quality Improvement (QI) Project
- Revenue Cycle Management (RCM) and Physician Practice Management (PPM) Assessment
- Telehealth Assessment
- Emergency Medical Service (EMS) Assessment
- Employee Engagement Assessment (EEA)
