The Emergency Medical Services Assessment identifies opportunities for improvements — including gaps in clinical services and the need for improved processes to enhance the coordination of services — at health care organizations participating in the Delta Region Community Health Systems Development Program.
Community care coordination (CCC) activities consist of events and workshops, check-in calls, learning collaboratives, training bootcamps, and an annual summit.
Eligible applicants include small rural hospitals (CAHs and PPS acute care facilities up to approximately 100 beds) RHCs, and other healthcare organizations located in Delta Regional Authority (DRA) designated counties and parishes.
