In this first step, Community Champions focus on building a comprehensive foundation for positioning their organizations to identify and connect with community partners.
This section of the toolkit is designed to support the Community Champion’s onboarding to the DRCHSD Program. Essential toolkit components include an outline of capacity-building learning activities, as well as customizable planning tools to host and facilitate a Community Connect Event. This toolkit is foundational to all steps of the community care coordination planning and development.

Enhancing Collaboration
Collaboration is essential to community care coordination planning and development. Community Champions are supplied learning opportunities that will empower community partners to have a role in understanding and addressing health needs, assessing resources, and identifying opportunities to improve health outcomes.
The DRCHSD Program requires Community Championsto participate in the following trainings during Step 1. Community Champion Q&A worktimes (mandatory and optional) are also offered on a weekly basis to address questions and apply the learning.

Lumina Self-Discovery

Lumina Spark is a personalized profile tool that provides a framework for better self-understanding and helps people identify how to improve their working relationships with others. Following completion of individual assessments, Community Champions will be assigned a Consultant Coach to debrief and understand their portrait. This will be followed by a team training that will help understand the dynamics of the team based on personality characteristics. One-hour 1:1 individual debriefs and 3-hour team training.
Capitalize on Your Capacity
Consultant Staff
This course helps you maximize your capacity by analyzing current engagement and anticipating true needs. Determine who needs to be part of the collaborative work, their level of involvement and commitment. Participants will learn how to understand the levels of involvement from partners and use this to strengthen collaboration and engagement. Identify who has the relationship to make the ask, the "why" that brings them to the table, how your collaboration’s "why" lines up with community partners’ "why", and the benefit of collaboration. One 3-hour session.
Conversations that Matter
Consultant Staff
Learn a conversation methodology that aligns with our natural way of thinking. Explore core facilitation principles and values in the development of an impactful and flexible facilitation style. Follow-up sessions will provide participants with a community of practice experience where they will have the opportunity to draft conversations, have additional practice, and get peer and coaching feedback. Facilitation Method: Focused Conversation. Two 3-hour sessions with two 2.5-hour follow-up sessions.

Mandatory Q&A worktimes will be used following the above trainings to ensure Community Champion application of all learning, where Community Champions can ask questions, practice, and network with others in a comfortable and safe environment.
Building Engagement and Encouraging Buy-In for Community Care Coordination Activities
The resources below should be utilized by the Community Champions as they start engaging with potential community partners, understanding the health needs of the area, and preparing for community care coordination. These will provide Community Champions with the necessary introduction script when meeting with potential community partners, and provide the why behind having them. There is also a worksheet that will help the Community Champion keep track of each potential community partner.
In Step 1, with the assistance of DRCHSD staff, Community Champions will be responsible for coordinating a two-hour Community Connect Event, answering the question "How can the health care organization connect with the community, build trust, and address concerns?" The Community Connect Event is built upon Technology of Participation (ToP) facilitation techniques that promote community engagement and participation to understand the needs of the community to address and improve health outcomes. DRHCSD staff use these ToP methods to bring community partners ‘all aboard’, build trust, and understand the community’s needs and concerns.
DRCHSD staff will work with Community Champions and supervisors to confirm event dates, schedule, and host planning calls. Community Champions will personalize the planning tools listed below for each respective health care organization's event. DRCHSD staff work behind the scenes to create each event agenda and facilitation to help prepare for the event.
The extent to which employees are passionate about their work and committed to the organization (engagement) and the choice by an employee to stay with an organization instead of seeking employment elsewhere (retention).
The DRCHSD Program (The Program) supports one Community Champion (CC) per selected organization to assist executive leadership with the Program goal of strengthening the local Health Care delivery system to position for population health.
These tools and resources will help the Community Champion monitor progress, strengthen collaboration and strategize for sustainability to ensure underserved populations benefit from coordinated care and support the facilitation of the Stay the Course Workshop.
