The National Rural Health Resource Center (The Center) has been awarded a five-year, $10 million cooperative agreement by the Health Resources and Services Administration’s (HRSA) Federal Office of Rural Health Policy to continue its management of the Delta Region Community Health Systems Development (DRCHSD) Program.

This presentation explores how integrating clinical and non‑clinical roles in scheduling and registration strengthens patient access, improves care coordination, and protects revenue in rural health clinics and rural hospitals.

The Delta Region Community Health Systems Development (DRCHSD) Program provides community care coordination (CCC) planning and strategic technical assistance to all program participants — with the goals of aligning local services with community needs, improving health outcomes and addressing social determinants of health.

The Delta Region Community Health Systems Development (DRCHSD) Program provides comprehensive technical assistance to participating health care organizations.

Financial and operational consultation services assess organization-wide operations at the department level of selected health care organizations participating in the Delta Region Community Health Systems Development (DRCHSD) Program to establish strategic priorities.

The DRCHSD Program (The Program) supports one Community Champion (CC) per selected organization to assist executive leadership with the Program goal of strengthening the local Health Care delivery system to position for population health.

The “Take Your Health in the Right Direction” campaign encourages patients to be advocates for their own health. By using supporting information and statistics to reinforce what could happen if regular appointments are missed or skipped, the campaign's audience is educated — and hopefully encouraged — to be more active in these important preventative health appointments that could save their lives.

The Telehealth Assessment identifies opportunities for telehealth implementation or expansion at health care organizations participating in the Delta Region Community Health Systems Development (DRCHSD) Program. 

The Emergency Medical Services Assessment identifies opportunities for improvements — including gaps in clinical services and the need for improved processes to enhance the coordination of services — at health care organizations participating in the Delta Region Community Health Systems Development Program.

The Delta Regional Authority announced 11 additional facilities chosen to participate in the Delta Region Community Health System Development (DRCHSD) Program.
