A report with the findings of the Summit has been developed to assist rural hospital leaders in engaging rural health providers in the transition to value-based purchasing and population health. This report is designed to help rural hospitals leaders and providers during the transition.
Use this series of leadership tools to enable rural health care leadership teams to examine and clarify roles for achieving performance excellence during the transition to value-based health care. They are geared specifically toward administrative teams, board members and physician leaders.

Speare Memorial Hospital (SMH), a critical access hospital located in Plymouth, New Hampshire, was one of six organizations selected to participate in the 2022-2023 Rural Health Provider Transition Project (RHPTP).
